Microorganisms isolated from blood cultures during the period of one year at the 82nd Year Rize State Hospital and their susceptibility to antibiotics

OBJECTIVE: The microorganisms which were isolated from blood cultures and their antibiotic susceptibilities during the between February 2010 and February 2011 in Microbiology Laboratory of Rize Central Hospital were assessed retrospectively, and aimed to contribute to antibiotic usage policies.
METHODS: : During this period, 900 blood culture samples were sent to our laboratory. Blood culture samples which send to our laboratory added to biphasic blood culture medium (Rosmedia, GBL) and were inoculated both sheep blood agar and EMB agar in accordance with routine procedures. After growth, microorganisms were identified with colony morphology and conventional methods which containing gram staining and biochemical tests. Antibiotic susceptibility was determined by Kirby-Bauer disk diffusion method according to the recommendations of Clinical and Laboratory Standards Institute (CLSI).


RESULTS: There was no growth in 750 of these samples (83.3% ) and 15 samples were (1.7 %) accepted as contamination. Various bacteria were determined in 135 samples (15.0 %); 108 of these organisms were (80.0 %) gram-positives, 23 were (17.0 %) gram-negatives and 4 were (3.0 %) Candida spp.
Coagulase Negative Staphylococcus (CNS) were the most frequently isolated species 94 (%87.0), followed by Staphylococcus aureus (S. aureus) 4 (%3.7), and Enterococcus spp. 10 (%9.3 ). Methicillin resistance were in 70.2 % of CNS and in 50.0 % of S.aureus. Glucopeptide resistance was not found in Staphylococcus and Enterococcus. The most frequently isolated Gram-negative species were Escherichia coli 6 (26.0%), Klebsiella species 5(21.7%), Pseudomonas spp. 5 (21.7%), Acinetobacter species 7(30.4%).
The most effective antibiotics were found as imipenem for the isolates of E.coli, and Klebsiella species and Pseudomonas spp. which were the most frequently isolated Gram-negative bacteria. But imipenem resistance was found 6(85.7%) of Acinetobacter spp. isolates which growth in the intensive care unit.


CONCLUSION: For determination of important bacteremia factors, the hospital staff should be given training on concerning blood collection techniques, the clinics should be informed about sending at least two blood culture bottles, and as a result of these, for studying more effectively, blood culture systems should be automated. Also, following the rates of antibiotic resistance would be in place approach for the determination of antibiotic treatment strategies because of changing the resistance of bacteria which important factors causing bacteremia.

AMAÇ: Şubat 2010 ve Şubat 2011 tarihleri arasında Rize 82.Yıl Devlet Hastanesi Mikrobiyoloji Laborotuvarına gönderilen kan kültür örneklerinden izole edilen çeşitli mikroorganizmalar ve antibiyotiklere duyarlılıkları retrospektif olarak araştırılarak antibiyotik kullanım politikalarına katkıda bulunulması amaçlanmıştır.
YÖNTEMLER: Bu süre içerisinde laboratuvara 900 kan kültürü örneği gelmiştir. Laboratuvarımıza gönderilen kan kültürü örnekleri bifazik Rosmedia (GBL), besiyerine ekilmiş ve rutin prosedürlerine uygun olarak EMB ve koyun kanlı agara pasajlanmıştır. Üreme sonrası mikroorganizmaların identifikasyonu koloni morfolojisi, Gram boyama ve biyokimyasal testleri içeren konvansiyonel yöntemlerle yapılmıştır. Antibiyotik duyarlılıkları Clinical and Laboratory Standards Institute (CLSI) önerilerine göre Kirby-Bauer disk difüzyon yöntemi ile belirlenmiştir.
BULGULAR: : Laboratuvara gelen kan kültür örneklerinin 750’sinde (%83.3) üreme olmamış, 15 örnek (% 1.7) kontaminasyon olarak değerlendirilmiştir. Örneklerin 135’inde (% 15.0) çeşitli mikroorganizmalar izole edilmiştir. Bu mikroorganizmaların 108’i (%80.0) Gram-pozitif bakteriler, 23’ü (% 17.0) Gram-negatif bakteriler ve 4’ü (% 3.0) Candida spp. olarak bulunmuştur. Gram-pozitif bakterilerden 94’ü (%87.0 ) koagülaz-negatif stafilokok (KNS), 4’ü (%3.7) S.aureus ve 10’u (%9.3) Enterococcus spp. idi.
Metisilin direnci KNS’da % 70.2, S. aureus’ta % 50.0 olarak bulunmuştur. Stafilokoklarda ve enterokoklarda glikopeptit direncine rastlanmamıştır.Gram negatif bakterilerden 6 (%26.0)’sı Escherichia coli (E.coli), 5 (%21.7)’i Klebsiella spp., 5 (%21.7)’i Pseudomonas spp., 7 (%30.4)’si Acinetobacter spp., olarak tanımlanmıştır. Gram-negatif bakterilerden en sık izole edilen E.coli, Klebsiella spp. ve Pseudomonas spp. izolatlarında en duyarlı antimikrobiyal imipenem olarak bulunmuştur. Yoğun bakım ünitesinde üreyen Acinetobacter spp. izolatlarının 6 (%85.7)‘sı imipeneme dirençli bulunmuştur.


SONUÇ: Bakteriyemi etkenleri arasında önemli yer tutan bakterilerin tanımlanması için kanın alınış tekniğinden başlanarak kan kültürleri konusunda hastane personeline eğitim verilmeli, en az iki şişe kan kültürü gönderilmesi konusunda klinikler bilgilendirilmeli ve bunların sonucunda kan kültürlerinin daha etkin çalışılması için otomatize sistemlere geçilmelidir. Ayrıca bakteriyemi etkenlerinin direnç profillerinde de zamanla değişim olduğundan antibiyotik tedavi stratejilerini belirlemek için bu etkenlerin antibiyotik direnç oranlarının takip edilmesi gerekmektedir.
