The effect of urinary catheter care bundle compliance on catheter-associated urinary tract infections

INTRODUCTION: Healthcare-associated infections (HAI) are a global problem of increasing importance all over the world. HAI’s, which are difficult to treat are possible to prevent. Therefore, the importance of infection control strategies has increased around the world. The application of several measures together (bundle) has been published more effective than the application of infection control measures separately. The purpose of this study was to assess the effects of the urinary catheter care bundle on catheter-associated urinary tract infections (CA-UTI).
METHODS: Between June 2015 and December 2020, the rates of urinary catheter care bundle compliance and the effects on CA-UTI rates were evaluated in intensive care units at XXX University Hospital. The diagnosis of CA-UTI was made according to the diagnostic criteria of the National Health Care-associated Infections Surveillance Guidelines. The urinary catheter care bundle consists of three parameters: 1. Continuation of the catheter insertion indication; 2. The catheter is below the level of the bladder; 3. Cleaning the catheter twice a day with a soapy sponge.
RESULTS: Urinary catheter care bundle compliance and standardized infection rates (SIR) of CA-UTI were assessed between June 2015 and December 2020. In 2015, bundle compliance was 0.80 (0.75-0.83), 0.88 (0.84-0.90) in 2016, 0.89 (0.87-0.91) in 2017, 0.90 (0.88-0.91) in 2018, 0.88 (0.85-0.89) in 2019, and 0.87 (0.84-0.88) in 2020. CA-UTI SIRs were 4.06 in 2015, 2.91 in 2016, 2.3 in 2017, 1.52 in 2018, 1.74 in 2019, and 2.81 in 2020. When the effect of the urinary catheter care bundle (in the years 2015-2018) is analyzed, it corresponds to a reduction of approximately 60%. "Catheter cleaning twice a day with a soapy sponge" was found to have the lowest compliance across all years.
DISCUSSION AND CONCLUSION: We observed that as bundle compliance increased, our CA-UTI rate decreased. However, the fact that our SIR values are >1 indicates that our infection control measures are inadequate. These findings imply that bundle components should be reviewed and/or that bundle compliance observation is lacking. Bundles can be used as an incentive to provide consistent and disciplined maintenance, but they are only one component of a larger improvement strategy. While main infection control systems (such as isolation, surveillance, and hand hygiene) are in routine, bundle applications should be integrated into these systems.

GİRİŞ ve AMAÇ: Sağlık hizmeti ilişkili enfeksiyonlar (SHİE) tüm dünyada giderek önemi artan küresel bir sorundur. Tedavisi zor olan bu enfeksiyonların önlenmesi mümkündür. Bu nedenle tüm dünyada enfeksiyon kontrol önlemlerinin önemi artmıştır. Literatürde enfeksiyon kontrol önlemlerinin ayrı ayrı uygulanması yerine birkaç önlemin birarada (demet) uygulanmasının daha etkin olduğu yayımlanmıştır. Bu çalışmada üriner kateter bakım demetinin kateter ilişkili idrar yolu enfeksiyonları (Kİ-İYE) üzerine etkilerini değerlendirmeyi amaçladık.
YÖNTEM ve GEREÇLER: XXX Üniversite Hastanesi’ndeki yoğun bakım ünitelerinde Haziran 2015-Aralık 2020 tarihleri arasında Kİ-İYE hızları ve bu hızlar üzerine üriner kateter bakım demeti uyum oranlarının etkisi değerlendirilmiştir. Kİ-İYE tanısı Ulusal Sağlık Hizmeti İlişkili Enfeksiyonlar Sürveyans Rehberi tanı kriterlerine göre konmuştur. Üriner kateter bakım demeti üç bileşenden oluşmaktadır: 1. Sonda takılma endikasyonunun devamı; 2. İdrar torbasının mesane seviyesinin altında olması; 3.Günde iki defa sabunlu sünger ile sonda temizliği.
BULGULAR: Haziran 2015-Aralık 2020 tarihleri arasında üriner kateter bakım demeti uyumu ve Kİ-İYE standardize enfeksiyon oranları (SIR) karşılaştırılmıştır. Demet uyumu 2015 yılında 0,80 (0,75-0,83) iken, 2016’da 0,88 (0,84-0,90), 2017’de 0,89 (0,87-0,91), 2018’de 0,90 (0,88-0,91), 2019’da 0,88 (0,85-0,89), 2020’ de ise 0,87 (0,84-0,88) olarak saptanmıştır. Kİ-İYE SIR değerleri ise 2015 yılında 4,06; 2016’da 2,91; 2017’de 2,3; 2018’de 1,52; 2019’da 1,74 ve 2020’de 2,81 olarak hesaplanmıştır. Üriner kateter bakım demetinin etkisi (2015-2018 yıllarında) analiz edildiğinde neredeyse %60’lık bir azalmaya denk gelmektedir. Tüm yıllarda en düşük uyumun olduğu bileşen “ günde iki defa sabunlu sünger ile sonda temizliği” olarak bulunmuştur.
TARTIŞMA ve SONUÇ: Demet uyumu arttıkça Kİ-İYE hızımızın düştüğünü kaydettik. Ancak SIR değerlerimizin >1 olması enfeksiyon kontrol önlemlerimizin yetersiz kaldığını göstermektedir. Bu sonuçlar bize demet bileşenlerinin gözden geçirilmesi gerektiğini ve/veya demet uyumu gözleminde eksiklik olduğunu düşündürmektedir. Demetler, disiplinli ve güvenilir bakım sağlamak için teşvik edici olabilir, ancak genel iyileştirme stratejisinin yalnızca bir bileşenidir. Enfeksiyon kontrolündeki temel sistemler işlerken (izolasyon, sürveyans, el hijyeni gibi), demet uygulamaları bu sistemlere entegre edilmelidir.
